Walk-In Appointments
Women's Health Practice sees walk-in patients on a limited basis and initially seen by our nurse practitioner. The best times are 8-9 AM, 12 noon to 1 PM, and 3:30-4:45 PM.
Annual Gynecology Services
Annual care is focused on your personal needs. Young women should get annual Pap smear, sexually transmitted disease prevention information, and contraception directed testing. As women pass into their 30s, issues of breast disease, gallbladder disease, and cardiovascular disease prevention become more of our focus. For women who are over 40 annual care may involve more thorough cancer screening, blood testing, and osteoporosis screening. After menopause, urologic and pulmonary diseases become more important. Ultimately, annual care will evolve over the years that we care for you. We encourage you to keep track of your concerns so we have time to address them.
Intravenous Infusion Capabilities
Intravenous infusion capabilities for new intravenous medications such as the osteoporosis medications Reclast and Boniva, and treatment for anemia as well as treatments for other conditions.
Evaluation of Gynecologic Problems
We are capable of doing medical and surgical evaluations of gynecological disease. We encourage you to have a copy of any previous records or treatments with you so we can avoid unnecessary test duplication. For many women, this may take several appointments. Women who have had symptoms with no firm diagnosis and no firm evaluation plan are encouraged to make an appointment for a consultation.
Surgical or Therapy Second Opinions
Most insurance companies no longer require second opinions, but Women's Health Practice feels strongly in extensive education before pursuing a surgical alternative. Often this means providing you with literature, references, or diagrams of the surgery you plan to undertake.
Chronic Pelvic Pain
Some women may feel pain daily, which affects their daily lifestyle. When pain has been persistent, it can affect your mental and physical health. You should seek the cause of this ongoing pain. Your physician may ask you questions about the pain, its effect on your life, and your emotions.
Endometriosis
The tissue that normally lines the uterus (the endometrium) grows somewhere else in the body, which may cause pelvic pain. The pain can range from mild to severe and may also be linked to infertility. Endometriosis symptoms can present as repeated, increasing discomfort/pain or a sharp deep pain, or some women experience no symptoms at all. Medical and surgical treatments will usually help patients with symptoms of endometriosis.
Pelvic Inflammatory Disease (PID)
Silent PID was one of the worst epidemics of the 20th century. Most women still do not know they were at risk. Many women with pelvic discomfort may be suffering from a variety of other conditions, such as endometriosis, chronic infections, ovarian cysts, or uterine fibroids. Most patients with PID have not had enough work-up to determine the accuracy of their diagnosis, and many women with prior PID treatment have not had adequate follow-up testing to make sure their problem has resolved.
Vaginitis/Vaginal Yeast Infections
Vaginitis is common in women of all ages. Vaginitis affects approximately 1/3 of women at sometime throughout their lives. Possible causes of vaginitis include infection, medication, irritating chemicals, systemic health problems (e.g. diabetes mellitus) or change in hormone balance. Women with vaginitis have abnormal discharge, itching, burning, foul odor, pain or irritation with intercourse or urination. Your doctor may examine a sample of the discharge under a microscope or culture the vaginal discharge. Treatment works best when matched with the cause of vaginitis. Treatment will consist of an appropriate antibacterial or antifungal medication.
X-ray Testing of the Fallopian Tubes
Special x-ray procedure, called hysterosalpingogram, in which a small amount of fluid is injected into the uterus and fallopian tubes to detect abnormal changes in their size and shape or to determine whether the tubes are blocked.
Vulvar Plastic Procedures
It is unusual for patients to need vulvar or vaginal procedures strictly for cosmetic purposes. More typical patients will need repair for specific anatomic defects. Women with these disorders may complain of a sensation of heaviness, discomfort with intercourse, or vaginal symptoms during the day.
Therapy and Alternate Solutions
We use some aromatherapies and do follow some of the recent scientific assessments of herbal therapies. We encourage patients to discuss their ideas, and we want to seek the least invasive and most natural solution to any problem. It is our job to inform you of possible drug and herb interactions, and to advise which herbal solutions are safest for each woman's health issue.

Obstetrics
Every aspect of your health is interrelated. Our sensitivity to your overall needs has led us to provide innovative and comprehensive obstetrical services. In order to serve our clients to the best of our ability, new obstetrics patients are only seen on a limited basis. Second opinion consultation visits are generally available.
Amniocentesis
An amniotic fluid sample is drawn and tested for certain genetic disorders. This test is performed in the doctor's office, and genetic amniocentesis is performed at 14-16 weeks of pregnancy.
Breastfeeding, Lactation, and It's Suppression
Breastfeeding is beneficial to both the mothers and babies. Breastfed babies have fewer illnesses and are 10% less likely to be admitted to the hospital during their first year of life. Medical research shows that breastfed babies benefit psychologically with higher scores on cognitive and IQ test. Breastfed babies have 50% less ear infections and three times less respiratory illness than bottle-fed babies. Mothers who breastfeed have increased protection against premenopausal breast cancer. Breastfeeding mothers also burn an increased 300-500 calories per day. The American Medical Association recommends breastfeeding for at least six months, preferably for one year. A good start begins with early mother-infant contact and proper positioning. Latch-on is important to prevent breast soreness and engorgement. You should expect to breastfeed every 1.5-3 hours; start 10 minutes on first breast; up to 20 minutes on 2nd breast. You should expect 6-8 wet baby diapers per day, 3-4 stools per day, and your baby should regain birth weight in two weeks. To suppress breastfeeding, you should wear a sports bra, avoid nipple stimulation, avoid hot water from shower, which may trigger letdown. Another option is to use cold cabbage leaves for the 1st 48 hours to decrease engorgement (breasts filling with milk) and tenderness.
Exercise in Pregnancy
Exercise is very important to you and your baby. If you stay active, you will feel better. Outdoor exercise and recreation gives you a chance to get sunshine and fresh air. Walking is particularly good because it strengthens some of the muscles you will use in labor and is important for a healthy psychological outlook. If you normally are active in sports, continue to enjoy them. However, it is wise to stop when you get tired. Also, try team activities instead of individual games. Avoid strenuous workouts. Do things with your friends and family. Swim, dance, go on a picnic, and participate in light sports that pose no danger of falling or being bumped. If you are thinking of trying a new sport or exercise or have been using a specific exercise routine, talk it over with your physician. Avoid lifting heavy objects and moving furniture while you are pregnant. Stretching will not harm you or your baby, but do not reach for things from a chair or ladder because you might lose your balance and fall. During the latter part of your pregnancy, you will probably begin to feel awkward because your balance is affected by your increasing size. At this point, you may want to substitute walking for more active sports.
Genetic Counseling
Two to four percent of the deliveries in the United States are complicated by birth defects, which may have a genetic base. Understanding if you are at risk and what screening tests are available to you and the baby's partner is an important part of prenatal care. Tay Sachs, Canavans, cystic fibrosis, and sickle cell anemia are the most common genetic diseases that we offer testing for. We can discuss ways to screen for genetic disease and provide referrals to other centers with more in-depth testing capability.
High-Risk Assessment
Our physician and facility are well suited to high-risk pregnancy assessment and complications. We have on site 3-D ultrasound or NST, body fat testing, rheography for deep vein thrombosis assessment, and the ability to do laboratory tests including fetal fibronectin testing and amniocentesis.
Gestational Diabetes
Gestational diabetes is a fairly common complication of pregnancy. In most cases, a woman who develops this condition has a family history of diabetes. The reason it shows up in pregnancy is that around the 26th or 27th week of pregnancy the placenta starts to produce a hormone that acts against your insulin. If you have inherited a tendency toward diabetes, your body may be unable to handle this challenge, and your blood sugar goes out of control. The extra blood sugar crosses the placenta, and your baby uses it to put on fat. When gestational diabetes goes untreated, the baby becomes very large, and delivery becomes difficult. However, in most cases, this condition is easy to treat. By following a special diet, tailored to the individual by our nutritionist, most gestational diabetic women can keep their blood sugar in normal range. Random blood sugar checks are done during regular office visits. Occasionally, a woman may need to check her blood sugar at home. Only rarely does a gestational diabetic require insulin shots. It is important, however, to follow the special diet for the health of both the mother and baby.
Fetal Heart Rate Monitoring (FHM)
Fetal heart rate monitoring measures the well being of the fetus in the uterus. Fetal monitoring cannot prevent a problem from occurring, but it can help alert your doctor to early warning signs.
Ultrasonography
An instrument measuring sound wave reflections is passed over the surface of the abdomen or internally to build a picture (sonogram) of the unborn child. The sonogram enables the doctor to determine the position and size of the baby, estimate due date, and spot some abnormalities. As of the printing of this book, the American College of Obstetrics and Gynecology (ACOG) does not recommend routine ultrasounds in pregnancy, but studies confirm routine ultrasound maybe helpful. We generally offer you a sonogram only if it is indicated for medical reasons. Some insurance plans mandate the timing and number of ultrasounds. We will help you understand and work within the guidelines of your insurance.
Nutrition in Pregnancy
Food plays an important role throughout your life in promoting a healthier you. Nutrition has an even greater importance to your health and your baby's health when you are pregnant. Eating wisely means choosing foods that provide you and your baby the protein, vitamins, minerals, and other essential nutrients that both of you need. Your body must increase blood, fluids, and tissue to develop your uterus and to prepare your breasts for breastfeeding. Protein, carbohydrate, fat, minerals, vitamins, water, and fiber are needed in the diet each day.

A hysteroscope (thin, telescope like instrument) is inserted into the vagina

Uterine Endometrial Ablation with Resection
An alternative to hysterectomy, endometrial is a procedure in which the lining of the uterus is destroyed in order to treat some cases.
Flexible Hysteroscopy
The newest way to diagnose gynecologic bleeding problems is with the Olympus flexible hysterofibrescope. This device, available at the Women's Health Practice and the Midwest Surgical Center has an insertion tube diameter of only 3.1 mm, allowing insertion without cervical dilation or anesthesia - and it is ideal for office hysteroscopy. For women who need additional local or iv sedation this is available as well.
NovaSure Endometrial Ablation
The NovaSure procedure is a quick, safe, simple, one-time endometrial ablation treatment which gently removes the lining of the uterus to control heavy bleeding. An advance in women's health, the ablation is completed in about 90 seconds of energy delivery and only needs to be performed once to lighten or stop your periods. Endometrial ablations have been offered at Women's Health Practice since the 1980's as Drs. Trupin and Campbell who brought the procedure to the area trained with pioneers of the technology. At Women's Health Practice office setting and the MidWest Surgical Center several technologies for ablation are offered for women who desire surgical control of their heavy menstrual bleeding. NovaSure is a minor procedure, most of our patients are done with just local and/or some sedation, allowing for both safety, and comfort and both the with a quick recovery time so you can go on with your life sooner.
Laparoscopic Supracervical Hysterectomy (LSH)
This is a technique of a completely laparoscopic hysterectomy, that Women's Health Practice has now brought to Champaign-Urbana. This surgery is the the least invasive, least traumatic, and the quickest way to remove the uterus. LSH should also be associated with an even lower risk of injury to the ureter or bladder (rare with hysterectomy) and without adverse effects to either vaginal support or sexual function. It may be performed with or without bilateral salpinoophorectomy (ovarian and tubal removal). The procedure has progressed rapidly with the new technology design of the newer generation of bipolar cautery instruments making heat even safer to use, and The GYNECARE Tissue Morcellator quickly and effectively extracts large volumes of tissue, which allows a surgeons to employ a minimally invasive approach that can help reduce operating time.
Pelvic Floor Reconstruction With New Mesh Technologies
The use of new mesh technologies offers a new level of success in repairs of the bladder in cases of stress incontinence, in repairs of the anterior vaginal wall, in cases of cystocele, and for repairs of the posterior vaginal wall, as in the cases of rectoceles. Symptoms of pelvic floor problems would include:
• Pressure and pain in the pelvic area or lower back.
• Urine leakage or difficulty in starting to urinate.
• Bowel problems, including constipation.
• Painful intercourse.
Other more complex cases can be handled by these technologies as well, and even uterine prolapse can be repaired in some cases without a hysterectomy. Some of the meshes used at Women's Health Practice and the Midwest Surgical Center are the Gynecare Prolift, Uretex® an the Avaulta BioSynthetic Mesh.
Urodynamic Testing
Accurate diagnosis is an important concern for the health care providers of Women's Health Practice, which is why we often recommend Uodynamic Testing prior to surgery. Surgical alternatives are available, and there are a variety of new designes of pessaries and some newer medications that may help patients as well.
Essure Procedure
The only completely effective sterilization procedure with no failures in any of the clinical trials. There is no cutting into the body, and most patients at Women's Health Practice and Midwest Surgical Center can have the procedure under local anesthesia either with or without sedation. You can have general anesthesia as well. The Essure procedure is different than the traditional method of a surgical laparoscopic ligation that can be done in our center as well. With Essure there is no cutting into the body. Instead, an Essure trained doctor inserts spring-like coils, called micro-inserts, through the body’s natural pathways.

Evaluation of the Abnormal Pap Smear
Colposcopy
An in-office procedure looking at the cervix through a microscope. This can detect changes that are invisible to the naked eye. This is done with a speculum and is no more uncomfortable than a Pap smear. If a biopsy is indicated, this may be performed at the time of colposcopy or recommended to be done by procedure at another time.
New pap smear tests with ThinPrep testing and new tests of viral exposure: The ThinPrep Pap Test, a new Pap test, is the first cervical cancer-screening test to be recognized by the U.S. Food and Drug Administration (FDA) as "significantly more effective" than the conventional Pap smear at detecting early signs of cervical abnormalities. Instead of smearing the cervical sample onto a slide as with a conventional Pap smear, your doctor rinses the cervical cell sample into a vial, which is then sent to a lab for slide preparation and review. Since the cells are not smeared, they do not clump together as they would on a slide. At the laboratory, a machine separates the cells and filters out the blood, mucus, and inflammatory cells. The important cells remain, and a representative portion of the entire sample is placed on a slide in a thin layer. Therefore, the result is a homogenized, representative "subsample", which is more clear and easier to read.

HPV Testing
Testing for the human papilloma virus (HPV) may well be the best bet yet for screening women at risk for cervical changes and cancer. Cervical cancer is an infectious disease, and HPV is the most common cause. All women are at risk for developing this problem, but the highest risk groups are young sexually active women and women with partners with the virus. The HPV assay test may be done from the same fluid used when a pap smear is taken or on a separate swab. The use of the information taken from this test, along with sexual history, pap smear results, and coloposcopy results (as warranted) aids your healthcare provider in giving you the most current picture of your cervical cancer risk.

Female sexual dysfunction is a complicated problem of sexuality difficulties in women and the causes can be vascular, neurological, hormonal/endocrine, or psychogenic. Much research needs to be done, but disorders are based on the following classifications. You may fit into more than one category. Placing a diagnosis on your condition will help us treat you.
Hypoactive sexual disorder: Decrease or absence of sexual thoughts, decreased interest in sexual activity, which causes personal stress or relationship difficulties.
Sexual aversion disorder: Aversion to and avoidance of sexual contact; may result from childhood trauma and/or physical or sexual abuse.
Hypoactive sexual desire disorder: Emotional or secondary to a medical problem, such as hormonal deficiency. May be due to menopause and endocrine disorder or other surgical or medical intervention.
Sexual arousal disorder: Inability to attain or maintain sexual excitement. May exhibit lack of lubrication or swelling.
Orgasmic disorder: Difficulty, delay, or inability to attain orgasm. Also includes primary (never orgasm) or secondary (due to trauma, illness, etc.).
Sexual pain disorder: Painful intercourse (dyspareunia or involuntary vaginal spasms (vaginismus). Vaginismus can be so dramatic that vaginal penetration may be painful.
Sexuality evaluation and education
Female arousal disorder: Inability to attain or maintain sexual excitement; may exhibit lack of lubrication or swelling.
Vaginismus: Involuntary vaginal spasms, which can be so dramatic that vaginal penetration may be painful.
Treatment of Anorgasmia
Anorgasmia is defined as the inability to achieve orgasm; although the term anorgasmia includes women who are medically unable to reach orgasm, the majority of anorgasmia are caused by psychological social, cultural, or relationship variables. Treatment includes both medical and therapeutic counseling.
Hormonal treatments: Include hormone replacement therapy, which is indicated for postmenopausal women, as well as other hormonal treatment options for your specific diagnosis.
